Accessing the Provider Portal

The Provider Portal allows you to:

  • Access Cozeva through single sign-on

Cozeva helps providers understand gaps in care, quality scores, and patient-level opportunities. Through Cozeva, you can:

Cozeva opens directly from the Provider Portal so you can move between systems easily.
