How to Apply
Joining Superior Choice Medical Group is simple.
Please click the button below to submit your Letter of Interest (LOI). After we receive your information, our Provider Relations team will reach out with the next steps for credentialing and contracting.
During the credentialing process, we review training, licenses, work history, and other required information according to NCQA standards.
After you’ve joined our network
Once credentialing and contracting are complete, your practice becomes part of the Superior Choice Medical Group network. You will have access to our contracted health plans, our Utilization Management team, and provider support services.
Our Provider Relations team will continue to support your practice with day-to-day needs and updates.
